Although these resources were developed specifically for carers week, they can be used at any time to help carers, particularly older carers, to identify themselves to primary care professionals.

An Activity Project for Carers of People with Dementia
This publication was developed by Carers Trust in Partnership with The Carers Centre for Brighton and Hove. Identification of Carers in GP Practices
One of the main obstacles to carers getting the right support is identification – both self-identification and identification by health professionals.
