Identifying and supporting unpaid carers in England to improve integrated system working

Carers Trust and Carers UK, as part of the Carers Partnership within the Health and Wellbeing Alliance, have co-developed a resource on supporting unpaid carers across health and social care in England and improving integrated system working.

This resource brings together key information, practical tools, and examples of best practices to help health and care professionals better support carers across health and social care systems.

It focuses on eight key areas of carer support:

  • Carer identification
  • Hospital discharge and carers
  • Social prescribing for carers
  • Carer contingency planning
  • Supporting carers in virtual wards
  • Integrated Care Systems (ICSs) engagement with carers
  • Carers’ breaks
  • Carers’ assessment

The resource will help commissioners and those providing services to better consider the needs of carers and ensure that carers receive the support they need.

Many of the materials included in this resource were developed as part of previous projects the Carers Partnership has collaborated on through the Health and Wellbeing Alliance.
