Involving Carers in Hospital Discharge
A person who is cared for may sometimes need admission to hospital. Unpaid carers who provide care at home need to be involved from admission onwards and kept updated about discharge plans.
We have worked with carers and NHS staff to develop two sets of guidance:
- policy guide for NHS service planners
- practical guide for frontline clinical staff
Resources for community pharmacies
Pharmacists and pharmacy staff are in a unique position to identify carers. At the heart of communities and often developing supportive relationships with regular customers.
